| 【摘要】 目的 分析研究湿润烧伤膏联合自体富血小板血浆在复杂性肛瘘术后创面中的应用效果。方法 选取 2021 年 1 月至 2022 年 3 月于南方医科大学第八附属医院接受手术治疗的 50 例复杂性肛瘘患者作为研究对象, 根据术后创面不同处理方式将其分为研究组 (25 例) 和对照组 (25 例)。研究组患者肛瘘术后创面采用湿润烧伤膏联合自体富血小板血浆治疗, 对照组患者肛瘘术后创面采用高锰酸钾溶液联合复方黄柏液涂剂治疗, 对比观察两组患者术后创面疼痛及瘙痒程度、腐肉完全清除及愈合时间。结果 术后第 4、14、24 天, 研究组患者术后创面疼痛程度均明显轻于对照组 (Z = -4.044、-4.083、-2.585, P<0.001、P<0.001、P = 0.010); 术后第14、24 天, 研究组患者术后创面瘙痒程度均明显轻于对照组 (Z = -4.122、-5.291, P 均<0.001); 研究组患者术后创面腐肉完全清除时间、愈合时间均明显短于对照组 (Z/ t = -6.127、16.571, P 均<0.001)。结论 与高锰酸钾溶液联合复方黄柏液涂剂治疗相比, 湿润烧伤膏联合自体富血小板血浆更能明显减轻复杂性肛瘘术后创面疼痛和瘙痒程度, 促进创面愈合。 |

| 【Abstract】 Objective To analyze the clinical efficacy of moist exposed burn ointment (MEBO) combined with autologous platelet?rich plasma in the treatment of complex anal fistula postoperative wounds. Methods 50 patients with complex anal fistula, admitted to The Eighth Affiliated Hospital of Southern Medical University from January 2021 to March 2022 for surgical treatment, were enrolled as research subjects and then divided into the study group ( n = 25) and the control group (n = 25) according to the different managements for postoperative wounds. After the surgical treatment for anal fistula, the postoperative wounds of patients were treated with MEBO combined with autologous platelet?rich plasma in the study group, whereas the patients in the control group were treated with the potassium permanganate solution combined with Compound Huangbai Lotion. Postoperative wound pain degree, itching degree, slough removal time, and wound healing time were compared between the two groups. Results Respectively on day 4, 14 and 24 after surgery, the wound pain degree of patients in the study group was significantly milder than that in the control group (Z = -4.044, -4.083 and -2.585, P< 0.001, P<0.001, P = 0.010). Respectively on day 14 and 24 after surgery, the itching degree of patients in the study group was significantly milder than that in the control group ( Z = - 4.122 and - 5.291, both P< 0.001). The complete slough removal time and wound healing time of patients in the study group were significantly shorter than that in the control group ( Z/ t = - 6.127 and 16.571, both P < 0.001). Conclusion Compared with potassium permanganate solution combined with Compound Huangbai Lotion, MEBO combined with autologous platelet-rich plasma is better in alleviating the wound pain and itching degree of complex anal fistula postoperative wounds, accelerating wound healing. |
